No-Shave Hair Transplant: What It Costs in Thailand and Who It Suits

Part of our hair transplant guide. Most people researching a transplant hit the same wall: the procedure works, but you have to walk around with a shaved head for weeks while it grows back. A no-shave transplant is the answer clinics sell to that, and it is real. It is also sold loosely. Two clinics can both advertise "no shave" and mean quite different operations, one of which still involves clippers.

This page covers what the procedure actually is, the two versions of it, the graft ceiling that comes with it, and what it costs at the Bangkok clinics that offer it. What a No-Shave Hair Transplant Is

The surgery itself does not change. Follicles are still taken one at a time from the back and sides of the head and placed into the thinning area, the same as any FUE procedure. What changes is that the surgical team works around your existing hair instead of clearing it out of the way first.

That sounds like a small difference. It is not. Shaving the head is not done for the patient's benefit, it is done because it exposes the angle, direction and exit point of every follicle, which is what lets a surgeon extract cleanly and match your natural growth pattern. Take that away and the same operation becomes slower and harder, and the trade-offs below all follow from that one fact.

The Two Versions, and Clinics Rarely Say Which One They Mean

This is the single most useful thing to know before a consultation, because the word "unshaven" covers both of these and the difference decides what you can actually have done.

Partial shave, sometimes called the donor window

A small rectangle at the back of the head is shaved, grafts are taken from inside it, and the longer hair above is combed down over the top. Nothing shows once you comb your hair. The recipient area is usually left alone or only the thinning patch is trimmed. This is what most clinics mean when they advertise no-shave, and it needs roughly 4 to 5cm of hair length to hide the window.

Fully unshaven

Nothing is trimmed anywhere. The surgeon extracts long hairs individually and places them between existing long hairs, usually with a DHI implanter pen so the tool can get between strands without cutting them. It is slow, it is the hardest version to do well, and it is normally kept for small jobs.

What to ask: "Is my donor area being shaved at all, and if so how large is the window?" A clinic that answers that clearly is telling you which procedure you are buying.

What It Costs at Bangkok Clinics

The one published comparison is the useful number here. Hairsmith charges ฿120 against ฿80, so the same clinic prices its non-shaven work 50 percent above its standard work. On a 2,000 graft job that is ฿240,000 instead of ฿160,000, a difference of ฿80,000.

That is one clinic's own figure for its own pricing, not a market average, but it brackets the Thai number rather than contradicting it.

Why It Costs More

Not materials. Time and difficulty. The surgeon is working with existing hair blocking the view, which means more care to avoid cutting through roots that are already there, and slower placement between strands. Clinicana puts an unshaven session at 7 to 9 hours against 6 to 8 for a shaved one, and that longer session is moving fewer grafts, not more. You are paying for theatre time and skill per graft, so the premium is charged per graft rather than as a flat fee.

The Graft Ceiling: the Real Trade-Off

Cost is the trade-off people expect. The ceiling is the one that actually rules patients out.

Grafts have a limited window outside the body, so a slower procedure cannot simply run longer to move the same volume. Clinicana caps unshaven sessions at 2,500 to 3,000 grafts and puts fully unshaven work at under 1,000 to 1,500, against 4,000 to 5,000 or more for a shaved session. Those are one clinic's published limits rather than an industry standard, but the constraint behind them applies everywhere.

Who It Actually Suits

Women with diffuse thinning. The clearest case. Female hair loss usually thins along the parting rather than clearing an area, so graft counts sit inside the ceiling anyway and there is plenty of length to hide a donor window. Shaving a woman's head to treat a thinning parting makes little sense.

People who are on camera or in front of clients. If your face is the job, a shaved head for six weeks is a real cost. Weigh the premium against what that time is worth rather than against the standard price.

Small, specific work. A receding corner, temple filling, or a touch-up on earlier work. Small graft counts are exactly where fully unshaven is practical.

Anyone with enough length to hide the window. Roughly 4 to 5cm. Short back and sides means there is nothing to comb over.

When a Standard Shave Is the Better Call

If you need 4,000 grafts or more, take the shave. Trying to move that volume unshaven means either several sessions or grafts spending too long out of the body, and the result is thinner than it should be. A few weeks of looking like you had a haircut is a small price against a density you keep for decades.

A clinic that tells you this unprompted is worth more than one that sells you whatever you asked for. If you raise heavy coverage and no-shave in the same sentence and nobody pushes back, that is a signal about the clinic.

Questions to Ask Before You Book

Get the price answer in writing against your own graft count. A published rate is a starting point, not a quote, and the per-graft structure means the total moves a lot with the number.
